  • CDC - NIOSH - The National Institute for Occupational Safety and Health. Providing National and World Leadership to Prevent Workplace Illnesses and Injuries.
  • US Dept. of Health and Human Services - The Department of Health and Human Services (HHS) is the United States government's principal agency for protecting the health of all Americans and providing essential human services, especially for those who are least able to help themselves.

State Governments

  • California: OHSEP - is a program in the California Department of Public Health that tracks work-related injuries and diseases.
  • New York: Dep of Labor - On page 2 specifies mandatory recording requirements by employers.
